City of Brantford Grant Evaluation Forms

Organizations that have received a grant from the City of Brantford Community Grants Program must submit a completed evaluation form at the end of the project. Please ensure you submit the evaluation for the correct grant stream. Submit your completed form to grants@brantcf.ca.

Note that you must submit your evaluation form prior to the next grant round opening to be eligible for future funding.
